Any student who participates in any Student Ministry activity with Christ Community during the 2022-2023 school year needs to have a Medical Release Form on file with us. A new form will need to be submitted for each new school year. This release form covers all activities during the school year & summer with the exception of retreats and missions trips, which have their own release form.
